Avoid standalone symptoms as final diagnoses when an etiology is supported.

• Specify acuity (acute, subacute, chronic, exacerbation)

• Specify severity (mild, moderate, severe)

• Link etiology (due to, from, secondary to)

• Add key qualifiers (laterality, with/without, initial/subsequent, presence of hypoxia/hypercarbia, SIRS/sepsis/septic shock, trauma mechanism, complications)
